Congratulations on a landmark trial ! My interpretation is that, while it is perfectly safe to operate as soon as possible , there is also no harm in delaying surgery until daylight hours on the day after admission (within 36 hours).I'm nervous about over-interpreting the sub-groups of patients who 'may' do better with very early surgery. However, I'm more nervous about the casual reader seeing no difference in the primary end-point and using this as an excuse to delay surgery indefinitely. The fact that the control group had surgery at an average of 24 hours seems very important. Am I over-thinking this? Matt Costa
