Eleven patients undergoing two-staged open reduction and internal fixation (ORIF) of an isolated pilon fracture were randomized to receive either 5ml of either autogenous leukocyte-reduce platelet-rich plasma (PRP) (n=5) or sterile 0.9% saline (n=6). The primary outcomes of this trial were to assess the number of post-traumatic osteoarthritis (PTOA)-related pro-inflammatory degradative biomarkers and anabolic biomarkers during the initial post-injury time period. The results of this trial illustrated that one intra-articular injection of leukocyte-reduced PRP is associated with anti-inflammatory, anti-degradative, and anabolic responses compared with the saline control. No adverse events of complications were reported.
