100 patients with musculoskeletal trauma were randomized to receive either a conservative transfusion strategy (transfusion at hemoglobin <5.5 g/dL; n=50) or a liberal transfusion strategy (transfusion at hemoglobin <7.0 g/dL; n=49). The primary outcome was the rate of infections, while secondary outcomes included deep vein thrombosis, renal insufficiency, and cardiac events. Outcomes were assessed at 30 days. Overall, the results revealed that the conservative group had no infections, while the liberal group had a 14% infection rate, with statistically significant differences noted. The study suggests that a conservative transfusion threshold is associated with a lower infection rate without increasing the risk of anemia-related complications.
