Full Weight Bearing Immediately After Uncemented Total Hip Arthroplasty: A Systematic Review of Randomized Trials .

Controversy persists regarding immediate full weight bearing (FWB) versus partial weight bearing (PWB) after uncemented THA, with concerns over prosthesis stability weighed against the potential for faster recovery. Across 13 RCTs, no recent trials in the past three years, meta-analysis showed that immediate FWB neither improved nor worsened key clinical outcomes—including HHS, Merle d’Aubigne scores, functional measures, or pain—compared with PWB. Narrative synthesis of safety outcomes similarly found no differences in femoral subsidence or loosening. However, evidence quality was rated very low due to bias, inconsistency, and imprecision, preventing firm conclusions and underscoring the need for larger, high-quality RCTs.
