Quadratus Lumborum Block for Postoperative Pain Management in Patients Undergoing Total Hip Arthroplasty: A Systematic Review and Meta-analysis
The latest look at quadratus lumborum blocks (QLB) for pain control after total hip arthroplasty shows a surprising level of consistency across seven recent RCTs. Whether comparing posterior or transmuscular approaches against sham procedures, other nerve blocks, or local infiltration analgesia, the overall story stays the same: pain scores, opioid use, and common side effects don’t differ in any meaningful way. Even subgroup analyses—focused only on QLB3 or only on sham-controlled trials—didn’t shift the picture. A few isolated findings favored alternatives such as femoral or fascia iliaca blocks, but the evidence base is still small and mostly low certainty. More high-quality trials are clearly needed before QLB can be recommended with confidence.
