The results of this study demonstrate that topical TXA reduces postoperative hemoglobin drops in patients undergoing hip fracture surgery. Limitations of the study include its single-centre nature and small patient population. Larger studies with active comparators are warranted.

Seventy four patients undergoing operative management for a hip fracture were randomized to receive intraoperative topical tranexamic acid (TXA) (n=31) or a placebo (n=34). Outcomes of interest were postoperative hemoglobin levels, transfusion rates and adverse events. The TXA group demonstrated significantly higher hemoglobin levels on postoperative days 1 and 2. When excluding intraoperative transfusions, no differences in postoperative transfusions were found between the two groups. No differences in adverse events were found between groups.
