137 patients with knee osteoarthritis (KOA) were randomized to placebo or resveratrol 500 mg/day for 12 weeks. Fifteen participants were lost to follow-up (5 placebo, 10 resveratrol), resulting in 122 patients analyzed: 65 in placebo and 57 in resveratrol. The outcomes included pain intensity (VAS), the Western Ontario and McMaster Universities Arthritis Index (WOMAC) score, knee flexion range of motion (ROM), and plasma silent information regulator factor 1 (SIRT1) levels. Other outcomes included Oxford Knee Score (OKS), Short Physical Performance Battery (SPPB), handgrip strength (HGS), gait velocity, C-reactive protein (CRP), and 8-isoprostanes. Outcomes were assessed at baseline and after 12 weeks. Overall, resveratrol significantly improved pain, functional performance, and plasma SIRT1 levels compared with placebo, with improvements in OKS, SPPB, HGS, gait speed, and reductions in oxidative stress markers.
