10 randomized controlled trials (RCTs) involving 591 patients with rotator cuff tendinopathy or partial-thickness tears were included in this systematic review and meta-analysis comparing platelet-rich plasma (PRP) versus corticosteroid (CS) injections. Pooled outcomes included pain measured using the visual analogue scale (VAS), shoulder function assessed using the American Shoulder and Elbow Surgeons (ASES) score and Constant–Murley score (CMS), and adverse events. At 3–6 weeks and 3 months, pain and functional outcomes were comparable between groups. However, PRP significantly improved pain and shoulder function at 6 months and was associated with fewer adverse events. These findings suggested that PRP provided modest but more sustained benefits than CS injections, although the clinical relevance of several improvements remained limited.
