61 patients with primary hip osteoarthritis undergoing total hip arthroplasty (THA) were randomized to receive either a short, collum-fixated Primoris femoral stem or a conventional metaphyseal-fixated and diaphyseal-engaging Echo stem. The primary outcome was periprosthetic bone mineral density (BMD), measured using dual-energy X-ray absorptiometry (DXA). Secondary outcomes included the European Quality of Life 5 Dimensions (EQ-5D), Oxford Hip Score (OHS), and Forgotten Joint Score-12 (FJS-12). Outcomes were assessed at 6 weeks, 12 months, and 24 months. Overall, the results of the study revealed that the Primoris stem preserved significantly more BMD in Gruen zones 3, 5, and 7 at 24 months, while patient-reported outcomes improved similarly between groups. These findings suggested that short, collum-fixated stems may offer advantages in femoral bone preservation without compromising clinical outcomes.
