Knee. 2026 07-Sep;():. 10.1016/j.knee.2026.104456Study Summary
15 studies including 3 241 049 patients with obesity undergoing primary total knee arthroplasty (TKA) were included in this systematic review and meta-analysis comparing patients with and without prior bariatric surgery (BS). Pooled outcomes included all-cause revision rates at 90 days, 1 year, and 2 years, while secondary outcomes included indications for revision, BS procedure type, and timing of TKA following BS. No significant differences in revision risk were observed between groups at any follow-up interval. Furthermore, BS procedure type did not consistently influence revision outcomes, although limited evidence suggested lower revision risks when TKA was performed within 2 years of BS. These findings suggested that prior BS did not reliably reduce revision risk following TKA and that its role in preoperative optimization required further investigation.
