Preoperative Intravenous Versus Oral Acetaminophen in Outpatient Surgery: A Double-Blinded, Randomized Control Trial

The results of this study suggest that oral and intravenous pre-operative acetaminophen provide similar post-operative pain outcomes, with no marked difference in adverse event rates in patients undergoing outpatient surgery. This study was limited by the performance of many different types of surgeries, resulting in significant differences in surgical protocols which may have affected outcomes, and lack of sub-group analyses by age, surgery type, and use of nerve block. Future randomized controlled trials are required to confirm the results of this study.

One hundred and twenty adults scheduled for an outpatient surgery were randomized to receive 1,000 mg of pre-operative intravenous acetaminophen (n=60) or oral acetaminophen (n=60) for post-operative pain relief. Outcomes of interest included the worst post-operative pain on a Numeric Rating Scale, incidence of opioid use, post-operative opioid consumption (in morphine milligram equivalents), the incidence of nausea, vomiting, and low O2 saturation, use of reversal agents, length of stay in the post-anesthesia care unit, and patient satisfaction with pain control. Results revealed no significant differences between the two groups in all outcomes (p>0.05 for all).
