Clinical Effectiveness of an Orthosis After Collagenase Clostridium Histolyticum Injection for Dupuytren Contracture.

Seventy-nine patients with Dupuytren contracture were randomized to receive a nighttime extension orthosis after collagenase clostridium histolyticum (CCH) (n=39) or no orthosis (n=40). The primary outcome of interest was residual flexion contracture (degrees) at follow-up. Secondary outcomes included QuickDASH and orthosis compliance; adverse events and skin tears were recorded. Outcomes were assessed post-manipulation and at 1 and 4 months. Overall, the results of the study revealed lower residual contracture with orthosis overall at 4 months, driven by a significant benefit at the proximal interphalangeal (PIP) joint, while metacarpophalangeal (MCP) joints showed no between-group differences. In short, post-CCH night orthosis appears beneficial for PIP contractures but not necessary for isolated MCP contractures.
