Liposomal bupivacaine infiltration in the surgical site for analgesia after rotator cuff repair: a randomized, double-blinded, placebo-controlled trial

The results of this study suggest that liposomal bupivacaine infiltration injections do not provide any significant reduction in pain relief or opioid consumption in patients undergoing arthroscopic rotator cuff tears. Whilst a significant difference in pain scores at 3 days post-operation was observed in favour of the bupivacaine group, this did not meet the threshold set for clinical significance. This study was limited by the performance of all procedures and injections by a single surgeon, which limits generalizability of results, as well as the small sample size and heterogeneity in local anesthetic used. Future studies with larger sample sizes and standard surgical protocols are required.

58 patients undergoing an arthroscopic rotator cuff repair were randomized to receive an infiltration surgical site injection of liposomal bupivacaine (n=27) or normal saline (n=27) for post-operative pain control. Outcomes of interest included numeric rating scale (NRS) pain scores after nerve block resolution and in the first 3 post-operative days, opioid consumption in the post-anesthesia care unit (PACU) and in the first 3 post-operative days, NRS pain scores during pendulum swing, patient satisfaction, and the incidence of adverse events. With the exception of pain scores on post-operative day 3, which was significantly lower in the bupivacaine group (p=0.003), all outcomes were not significantly different between the two groups (p>0.05 for all).
