Nineteen studies including 391 964 patients with obesity undergoing joint arthroplasty were included in this systematic review and meta-analysis comparing prior bariatric surgery versus no bariatric surgery. Pooled outcomes of interest included periprosthetic infection, periprosthetic fracture, dislocation, revision rate, mortality, and need for blood transfusion. Overall, bariatric surgery showed no significant effect on infection, fracture, revision, mortality, or transfusion rates, but was associated with increased risk of dislocation in total hip arthroplasty (THA) patients. These findings suggest bariatric surgery does not substantially alter most long-term arthroplasty outcomes but may influence hip stability.
