Supervised or Home-Based? Exploring the Best Exercise Approach for Knee Osteoarthritis Management: A Systematic Review and Meta-Analysis.
J Clin Med . 2025 Jan 15;14(2):525.Ten studies including 917 patients with radiographically confirmed knee osteoarthritis were included in this systematic review and meta-analysis comparing supervised exercise programs to home-based exercise programs. Pooled outcomes of interest included pain, physical function/disability, gait performance, and exercise adherence. Supervised exercise was significantly more effective in reducing pain and disability than home-based programs. However, a dose–response relationship was found for home-based interventions, indicating that increased frequency and duration were linked to better outcomes. These findings suggest that while supervised programs are superior, optimized home-based programs can still be clinically effective.
