149 scheduled for primary total knee arthroplasty (TKA) were screened, and 120 patients were randomized to receive tranexamic acid (TXA) alone, absorbable hemostat (AH) alone, or tranexamic acid combined with absorbable hemostat (TXA+AH). The primary outcome of interest was perioperative blood loss, hemoglobin and hematocrit levels, maximum hemoglobin change, and postoperative complications. Secondary outcomes were perioperative inflammation, coagulation function, and early knee function. Outcomes were assessed through the perioperative period and during follow-up exceeding 6 months. Overall, the results of the study revealed significantly lower total and hidden blood loss, higher postoperative hemoglobin levels, and reduced anemia rates in the combination group compared with either single-treatment group, without an increase in complications. These findings suggest that combining tranexamic acid with absorbable hemostat provides superior blood management and accelerates early recovery after TKA.
