Effectiveness of intra-articular vancomycin in preventing prosthetic joint infections in hip and knee arthroplasty: A systematic review and meta-analysis of RCT's.

Five randomized controlled trials involving 1485 patients undergoing primary hip or knee arthroplasty were included in this systematic review and meta-analysis comparing intra-articular vancomycin powder with standard prophylaxis alone. Pooled outcomes of interest included prosthetic joint infection rates, adverse drug reactions, and deep vein thrombosis. The infection rate was lower in the intervention group, but this difference was not statistically significant. Adverse events were also similar, and no significant difference was found for deep vein thrombosis. The analysis suggests that while there may be a trend toward a lower infection rate with intra-articular vancomycin, further research with larger sample sizes is needed to confirm these findings.
