Prospective Multicenter Randomized Controlled Trial Comparing Early Protected Movement and Splinting for Fifth Metacarpal Neck Fracture

Splinting and elastic bandaging with early protective movement provided similar hand-related health status and grip strength in patients with fractures of the 5th metacarpal neck. This study was limited by the small sample size that failed to reach target recruitment. Moreover, an attrition rate of >20% per month was observed. Multicenter studies with larger sample sizes are needed to confirm the results of this study.

Thirty-seven patients with 5th metacarpal neck fractures were randomized to receive elastic bandaging with early protective movement (n=16) or splinting (n=21). The outcomes of interest included the Brief Michigan Hand Questionnaire (bMHQ) and grip strength, as well as cost of intervention. Outcomes were assessed up to 12 weeks follow-up. No significant differences were observed between the two groups in both bMHQ scores and grip strength. The cost of intervention per patient was 0.50$ in the elastic bandaging group vs. 3.98$ in the splinting group.
