Double-bundle anterior cruciate ligament reconstruction resulted in better International Knee Documentation Committee objective grading at fifteen year follow-up compared to single-bundle reconstruction.

One hundred fifty-three patients with ACL injuries were randomized to receive single-bundle (n=78) or double-bundle (n=75) ACL reconstruction using hamstring autografts. The primary outcome was the objective grading of the International Knee Documentation Committee (IKDC). Secondary outcomes included knee stability, range of motion (ROM), subjective assessments, and functional tests. Outcomes were assessed at 15 years. Overall, the study revealed that the double-bundle technique significantly improved IKDC objective grading, while other outcomes showed no statistically significant differences. The findings suggest that double-bundle reconstruction offers long-term advantages in stability-related outcomes.
