Comparison of total blood loss between limited tourniquet use and conventional tourniquet use in total knee arthroplasty: a randomized controlled trial.

Ninety patients undergoing unilateral primary TKA were randomized to receive either limited tourniquet use (LIM-TKA, n=45) or conventional tourniquet use (CON-TKA, n=45). The primary outcome of interest was total blood loss. Secondary outcomes included the need for blood transfusion, wound grading scores, incidence of venous thromboembolism (VTE), length of hospital stay, and reoperation rates. Outcomes were assessed up to the follow-up period, with wound evaluations performed in the second postoperative week. Overall, the results revealed that the LIM-TKA group had significantly lower total blood loss, better wound grading scores, and a shorter hospital stay compared to the CON-TKA group. The need for blood transfusion, incidence of VTE, and reoperation rates were similar between groups. These findings suggest that limited tourniquet use in TKA may reduce total blood loss and improve postoperative recovery without increasing complication rates.
