Comparing proximal and distal chevron osteotomy for moderate to severe hallux valgus: A meta-analysis of clinical and radiological outcomes, focusing on hallux valgus angle and intermetatarsal angle.

4 studies including 294 cases with moderate to severe hallux valgus deformity were included in this meta-analysis comparing proximal chevron osteotomy (PCO) vs distal chevron osteotomy (DCO). Pooled outcomes of interest included hallux valgus angle (HVA), intermetatarsal angle (IMA), American Orthopaedic Foot & Ankle Society (AOFAS) scores, postoperative complications, and radiological correction. Overall, there were no significant differences between PCO and DCO for postoperative HVA, IMA, or AOFAS scores. Both procedures demonstrated favourable clinical and radiological outcomes with low complication rates, although some minimally invasive proximal techniques showed improved deformity correction in individual studies. These findings suggest that DCO may serve as a reliable alternative to PCO for appropriately selected patients with moderate to severe hallux valgus deformity.
