Randomized clinical trial comparing pericapsular nerve group (PENG) block and periarticular local anesthetic infiltration for total hip arthroplasty.

Sixty patients undergoing primary total hip arthroplasty were randomized to receive either a pericapsular nerve group (PENG) block (n=30) or periarticular local anesthetic infiltration (PAI) (n=30). The primary outcome was the incidence of quadriceps motor block at 3 hours postoperatively. Secondary outcomes included pain scores (static and dynamic) up to 48 hours, morphine consumption, time to opioid request, motor and sensory block assessments, physiotherapy performance, opioid-related side effects, and length of hospital stay. Overall, the results of the study revealed no significant difference in quadriceps weakness between groups. However, PAI was associated with lower static pain scores at all time intervals and lower dynamic pain scores at 3 and 6 hours. These findings suggest that while motor block rates were similar, PAI may offer superior early postoperative analgesia.
