On-Arrival Lumbar Epidural Analgesia for Lower Limb Trauma Pain in Emergency Care: A Superiority Randomized Controlled Trial.
Cureus. 2024 Feb 21;16(2):e54622.Three hundred fifty-six patients with lower limb trauma were randomized to receive either epidural analgesia (ELETRA) (n=157) or standard pain management (n=160). The primary outcome was pain relief, measured by a visual analog scale (VAS), with secondary outcomes including stress markers (CRP, IL-6, and cortisol), hemodynamic stability, adverse effects, and patient satisfaction. Outcomes were assessed at baseline, 1, 6, 12, and 24 hours post-intervention, and up to 1 week. Overall, the study revealed that ELETRA significantly improved pain control, reduced stress markers, and led to higher patient satisfaction compared to standard care. The findings suggest ELETRA is a superior pain management strategy for lower limb trauma in emergency settings.
