Thirty patients with ACL rupture were randomized to receive either conventional ACL reconstruction without biologic augmentation (n=10), platelet-rich plasma treatment (PRP; n=10), or a combination of bone marrow aspiration concentrate (BMAC) and PRP (n=10). Outcomes of interest were graft maturation and tendon-bone tunnel healing as assessed by MRI, functional scores (Lysholm knee score and the International Knee Documentation Committee (IKDC 2000)), knee laxity, and peri-tunnel edema. Outcomes were assessed preoperatively and at 12, 24, and 48 weeks post-surgery. Overall, the study revealed that the BMAC + PRP group had significantly improved knee laxity at 24 weeks compared to the control group. However, no significant differences were observed in graft maturation and functional scores among the groups. The findings suggest that while BMAC + PRP may improve knee stability, it does not significantly enhance graft maturation or tendon-bone healing compared to PRP alone or no augmentation.
