Bankart repair with remplissage significantly reduces redislocation and recurrent instability rates compared to Bankart repair alone in patients with engaging Hill-Sachs lesions, supporting its use for improved medium-term outcomes in shoulder instability. However the study’s reliance on self-reported outcomes and variable follow-up times are limitations.

One hundred and eight patients with recurrent anterior shoulder instability and an engaging Hill-Sachs lesion were randomized to receive either arthroscopic Bankart repair with remplissage (REMP, n=52) or isolated Bankart repair (NO REMP, n=50). The primary outcome was failure, defined as a redislocation, and overall recurrent instability (redislocation or ≥2 subluxations) was also assessed. Outcomes were measured at a mean follow-up of four years. Overall, the study found that the REMP group had significantly lower rates of redislocation and recurrent instability compared to the NO REMP group.
