Analgesic Efficacy of Gabapentin in Patients Undergoing Carpal Tunnel Release Surgery: A Systematic Review and Meta-Analysis of Randomized Placebo-Controlled Trials.

Clinicians should note that gabapentin, particularly at higher doses, can reduce postoperative pain at 6 to 24 hours following carpal tunnel release surgery without increasing the risk of side effects. This suggests a beneficial role for gabapentin in early postoperative pain management. However, the study’s short follow-up (24 hours), varied dosing and administration timing, and single-center nature of included trials limit the generalizability of results.

Three randomized controlled trials including 205 patients with carpal tunnel syndrome were included in this systematic review and meta-analysis comparing perioperative gabapentin versus placebo. Pooled outcomes of interest included postoperative pain at one, six, 12, and 24 hours, and the incidence of adverse events. Gabapentin significantly reduced postoperative pain at six, 12, and 24 hours, but not at one hour. Adverse events such as dizziness, drowsiness, tinnitus, and metallic taste were similar between groups. These findings suggest gabapentin is effective in reducing early postoperative pain without increasing adverse effects.
