Does Combined Intra- and Extraarticular ACL Reconstruction Improve Function and Stability? A Meta-analysis

Clin Orthop Relat Res. 2015 Aug;473(8):2609-18

8 randomized control trials (RCTs) comparing the use of intra-articular reconstruction alone with the combined use of both extra-articular and intra-articular reconstruction for the treatment of a ruptured anterior cruciate ligament (ACL) were selected for inclusion in this meta-analysis. The purpose of the study was to determine if the addition of extra-articular reconstruction to the standard interior reconstruction affects knee function and stability. Secondary outcome measures were the comparative risks of complication between the two treatment options. Pooling of results indicated improved knee stability (significant improvement in pivot-shift and Lachman test scores) for patients receiving both types of reconstruction, however, no significant differences in tests of overall functional outcome were noted. The difference in complication risk between groups was insignificant.
