With advancements in arthroscopic techniques, there’s been renewed interest in anterior cruciate ligament (ACL) repair as an alternative to the long-standing gold standard of ACL reconstruction. Evidence comparing arthroscopic ACL repair with ACL reconstruction was updated, adding one new paper since 2020. Across four RCTs, most outcomes showed no significant differences between procedures. Overall certainty of evidence was low due to small sample sizes, imprecision, and limited follow-up. While modern repair techniques such as DIS and BEAR show promise, longer-term, larger trials are needed to clarify true comparative effectiveness and identify which patients may benefit most.
