Questions This Issue Explores

In patients with intra-articular distal humerus fractures, does open reduction and internal fixation result in different elbow function, range of motion, surgical time, hospital stay, complications, infections, and revision surgery compared with total elbow arthroplasty?
In adults with acute Achilles tendon rupture, does individualized treatment guided by the Copenhagen Achilles tendon Rupture Treatment Algorithm, compared with routine non-operative or operative treatment, improve functional performance at 12 months?
In adults undergoing foot and/or ankle surgery, does postoperative prophylactic mecobalamin, compared with placebo, reduce the occurrence of CRPS-1 and improve pain and functional outcomes over 12 months?
In adults undergoing primary TKR, does patellar resurfacing, compared with no patellar resurfacing, improve knee function, quality of life, reoperation rates, and cost-effectiveness over 20 years?
In adults undergoing open reduction and internal fixation for closed calcaneal fractures, does wound closure using the Allgöwer-Donati technique reduce suturing time, drainage tube duration, and postoperative incisional complications compared with interrupted vertical mattress suturing during short-term postoperative follow-up?

The Invisible Opponent: How Travel Fatigue and Jet Lag Challenge Athletic Performance

For elite athletes, travel is more than a logistical challenge—it can directly affect performance. Long flights, changing time zones, disrupted sleep, dehydration, prolonged sitting, and unfamiliar environments can contribute to jet lag, fatigue, illness, slower reactions, and impaired decision-making. These effects will be especially relevant during the 2026 FIFA World Cup, as teams travel between Canada, Mexico, and the United States while maintaining demanding competition schedules. Strategies such as adjusting sleep before departure, timing light exposure, staying hydrated, using caffeine or melatonin appropriately, and maintaining strong hygiene practices may help athletes adapt more effectively. In a tournament where small performance differences can determine the outcome, managing travel could become an important part of competitive success.

Clinical and economic results of patellar resurfacing in total knee replacement

1715 patients undergoing primary total knee replacement (TKR) were randomized to receive patellar resurfacing or no patellar resurfacing. The primary clinical outcome was the Oxford Knee Score (OKS), while the primary economic outcome was incremental cost per quality-adjusted life-year gained. Secondary outcomes included the 12-Item Short Form Health Survey (SF-12), European Quality of Life 5-Dimensions 3-Levels score (EQ-5D-3L), reoperations, complications, health-care costs, and mortality. Outcomes were assessed at 3 months, 1 year, and annually for up to 20 years. Overall, the results revealed no statistically significant difference in OKS or other clinical outcomes, although most estimates numerically favoured patellar resurfacing. Patellar resurfacing produced significantly more quality-adjusted life-years without increasing overall 20-year health-care costs, suggesting that it is likely the preferred and more cost-effective strategy.
