Robust evidence for app-based intervention in axial spondyloarthritis .
Can a smartphone app meaningfully shift the trajectory of axial spondyloarthritis management? The Bechterew App Trial demonstrated that a disease-specific digital intervention significantly improved patient-reported disease activity, physical function, and quality of life compared with usual care, with markedly higher response rates across key indices. Meaningful gains were also observed in global pain, patient global assessment, self-efficacy, and adherence to prescribed exercise, highlighting the potential of structured digital support to reinforce guideline-based nonpharmacologic care. Engagement with the app was consistently high, and no device-related safety concerns emerged, supporting feasibility and acceptability in routine practice. Nevertheless, interpretation warrants caution given the unblinded design, reliance on subjective outcomes, absence of inflammatory biomarkers, and relatively short follow-up period. Despite these limitations, the findings provide persuasive early evidence that scalable, app-based interventions may enhance long-term disease management and help bridge the persistent gap between recommended exercise therapy and real-world adherence in axSpA.
