Two-stage ligament reconstruction with remnant preservation as treatment of knee dislocation.

Seventy patients with multi-ligament knee dislocation were randomized to receive two-stage reconstruction with remnant preservation (n=48) or two-stage simple reconstruction without remnant preservation (n=22). The primary outcome of interest was knee function (IKDC, Lysholm, Tegner) and stability (Lachman/Drawer), with range of motion and imaging as additional endpoints. Outcomes were assessed from 12 months postoperatively with follow-up 24–91 months (mean ≈60 months). Overall, the results of the study revealed that both groups improved substantially from baseline, but remnant preservation achieved higher final knee flexion and superior IKDC, Lysholm, Tegner scores, and fewer positive Lachman tests. These findings suggest that, within a staged strategy, preserving ACL/PCL remnants enhances clinical recovery without added complications.
