337 patients with knee osteoarthritis were randomized across 5 randomized controlled trials to receive metformin-based therapy or placebo/usual care control therapy. The primary outcome of interest was pain. Other outcomes of interest included joint stiffness, physical function, health-related quality of life, patient global assessment, and harms. Outcomes were assessed up to 4 to 24 weeks. Overall, the results of the study revealed that metformin was associated with significantly greater improvements in pain, stiffness, physical function, and quality of life compared with control treatment. These findings suggest that metformin may provide symptomatic benefit as an additional option for knee osteoarthritis, although the evidence does not yet prove true disease modification.
