Seventy-nine patients with surgically repaired traumatic full-thickness supraspinatus tears were randomized to receive either progressive exercise therapy (n=38) or usual care (n=41). The primary outcome of interest was the structural comparison of supraspinatus tendon thickness and acromiohumeral distance (AHD) between repaired and contralateral shoulders. Secondary outcomes included supraspinatus muscle thickness, neovascularization, and effects of tendon retear or limb dominance. Outcomes were assessed at the 1-year follow-up. Overall, the study revealed that the repaired shoulder had a significantly thinner supraspinatus tendon and reduced AHD compared to the contralateral shoulder across both treatment groups, with no significant impact from early progressive exercise.
