2026 SICOT Annual Meeting: Kinematic alignment in robotic total knee arthroplasty: A randomized comparative study of five-year outcomes

100 patients with varus knee osteoarthritis (Kellgren-Lawrence grades 2–3) were randomized to receive robotic total knee arthroplasty (RoTKA) with restricted kinematic alignment (rKA) (n=50) or mechanical alignment (MA) (n=50). The primary outcome of interest was not explicitly identified. Outcomes included range of motion (ROM), Knee Society Score (KSS), Oxford Knee Score (OKS), 36-Item Short Form Health Survey (SF-36), and 12-item Forgotten Joint Score (FJS-12). Outcomes were assessed at 3 months and annually for 5 years postoperatively. Overall, the results revealed that rKA was associated with improved early functional scores and superior SF-36 physical component and FJS-12 scores at 5 years compared with MA. No revision surgeries occurred in either group. These findings suggest that rKA may provide modest functional advantages over MA in RoTKA without compromising mid-term implant survival.
