2025 SICOT Annual Meeting: Application Of A Modernized Plate With Bicortical Screw Fixation And Serclage Integration Through Transverse Foramen In The Treatment Of Periprosthetic Fractures Of The Proximal Femur Type B2 Of The Vancouver Classification

Thirty patients with periprosthetic proximal femur type B2 fractures were randomized to undergo fixation with a newly developed anatomical plate (n=15) or a standard plate (n=15). The primary outcome of interest was hip function measured by Harris Hip Score (HHS). Secondary outcomes included Oxford Hip Score (OHS), Timed Up-and-Go test, radiographic fracture healing, operative time, blood loss, and complications. Outcomes were assessed at 1, 3, 6, and 12 months. Overall, the results revealed that the new plate significantly improved HHS at 3 months (p=0.038) and 12 months (p=0.044) and OHS at 3 months (p=0.026). Surgery was shorter (−17.5 minutes, p=0.005) and blood loss was lower (−71 ml, p=0.003) with the new plate. Complication rates were similar between groups. These findings suggest that the newly developed plate may provide superior functional recovery and perioperative efficiency compared to standard plates.
