2025 SICOT Annual Meeting: Conversion Total Hip Arthroplasty After Failed Sliding Hip Screw Compared To Cephalomedullary Nail Failure: A Systematic Review And Meta-Analysis

Three retrospective cohort studies including 680 patients with failed intertrochanteric fracture fixation (327 CMN, 353 SHS) were analyzed in this systematic review and meta-analysis comparing CTHA outcomes. Pooled outcomes included medical complications, orthopaedic complications, peri-prosthetic fractures, and Harris Hip Scores (HHS). SHS failures had a higher risk of orthopaedic complications (RR 1.64, 95% CI 1.18–2.29), but no significant differences were found in medical complications, peri-prosthetic fractures, or HHS compared with CMN failures. Overall, CTHA provided similar functional recovery regardless of fixation type, though SHS was associated with greater orthopaedic complication risk.
