Short Versus Regular Periods of Cast Immobilization for Distal Radial Fractures: A Systematic Review and Meta-Analysis.

Three randomized controlled trials including 252 patients with distal radial fractures were included in this systematic review and meta-analysis comparing short (3-4 weeks) versus regular (5-6 weeks) cast immobilization periods. Pooled outcomes of interest included the shortened disabilities of the arm, shoulder, and hand questionnaire (qDASH) scores, patient-rated wrist evaluation (PRWE) scores, visual analog scale (VAS) score after cast removal, the incidence of total complications, and the incidence of complex regional pain syndrome (CRPS). Results demonstrated significantly better qDASH scores in the short-duration group at 12 weeks, six months, and one year, with PRWE scores also favoring the short-duration group at one year. No significant differences were noted in VAS scores, total complications, or CRPS occurrence. These findings suggest that shorter periods of cast immobilization can lead to better patient-reported functional outcomes without increasing complications, providing a basis for potentially revising current clinical guidelines.
