Safety of Lidocaine During Wide-Awake Local Anesthesia No Tourniquet for Distal Radius Plating.
J Hand Surg Glob Online. 2023 Jan 9;5(2): 196-200.24 patients undergoing a distal radius plating surgery for fracture were randomized to receive lidocaine plus adrenaline during a wide-awake local anesthesia no tourniquet procedure (n=19) or lidocaine only (n=5). The outcomes of interest included duration of numbness, serum lidocaine levels, and lidocaine toxicity. The combination of lidocaine plus adrenaline significantly extended the duration of numbness and increased serum lidocaine levels up to 6 hours, without reaching the point of toxicity. The biochemical safety of lidocaine with adrenaline should be confirmed in larger, balanced randomized trials.
