Remimazolam & Sufentanil on Anesthetic Induction in Elderly Hypertensives Undergoing Surgery .
Effects of remimazolam combined with sufentanil on hemodynamics during anesthetic induction in elderly patients with mild hypertension undergoing orthopedic surgery of the lower limbs: a randomized controlled trial.
BMC Anesthesiol . 2023 Sep 14;23(1):311.60-patients between the age of 65-and-75 years with mild hypertension who were undergoing elective lower limb orthopedic surgery under general anesthesia were randomized to receive either remimazolam besylate at 0.2 mg/kg (Group R) (n = 30) or propofol at 1.5 mg/kg (Group P) (n = 30). The primary outcomes of interest were the disappearance time of the eyelash reflex, mean arterial pressure (MAP), heart rate (HR), Cardiac output (CO), Cardiac Index (CI), and systemic vascular resistance (SVR). The secondary outcome of interest was the incidence of any adverse events which included hypotension, bradycardia, hiccupping, nausea, injection pain, and vomiting. Outcomes were measured before induction (T0), when the eyelash reflex disappeared (T1), immediately after endotracheal intubation (T2), 1 min after endotracheal intubation (T3), and 5 min after endotracheal intubation (T4). The results of the study showed that the time for disappearance of eyelash reflex was significantly longer in group R. It was also seen that with respect to baseline, MAP, HR, CO, CI and SVR were more stable at all time points in Group R while they were significantly deviated in Group P with a more profound effect. Importantly the number of adverse events was significantly lower in group R ( 20%vs 73%).
