Platelet-rich plasma for the treatment of discogenic low back pain: a prospective randomized controlled trial.
Front Pain Res (Lausanne) . 2025 Aug 13:6:1648772Forty patients with discogenic low back pain were randomized to receive platelet-rich plasma (PRP) injections (n = 20) or methylene blue injections (n = 20). The primary outcome was pain (VAS). Secondary outcomes included lumbar function (JOA), Pfirrmann grade, and MRI apparent diffusion coefficient (ADC) values. Outcomes were assessed before treatment and at 6 months. Overall, the results showed that PRP led to significantly greater pain reduction and functional improvement than methylene blue, with PRP also improving disc ADC and reducing disc degeneration grades. These findings suggest that PRP may offer both symptomatic and structural benefits in discogenic low back pain.
