Percutaneous pedicle screw combined with percutaneous kyphoplasty for osteoporotic vertebral compression fractures with endplate depression: A randomized controlled trial.

128 elderly patients with single-level osteoporotic vertebral compression fractures with endplate depression were randomized to receive percutaneous pedicle screw fixation combined with percutaneous kyphoplasty (PPSF + PKP) or percutaneous kyphoplasty alone (PKP). The outcomes of interest were radiographic restoration and maintenance of vertebral alignment, including Cobb angle, anterior vertebral height, and central vertebral height. Other outcomes included pain - visual analogue scale (VAS), functional disability (Oswestry Disability Index [ODI]), secondary loss of correction, and postoperative complications. Outcomes were assessed preoperatively and at 2 days, 3 months, 1 year, and 2 years postoperatively. Overall, the results of the study revealed that PPSF combined with PKP achieved superior vertebral height restoration, better maintenance of correction, and improved long-term functional outcomes compared with PKP alone, while maintaining a similar complication profile. These findings suggest that PPSF combined with PKP is a safe and more effective minimally invasive treatment for osteoporotic vertebral compression fractures with endplate depression.
