Is the Combination of Platelet-Rich Plasma and Hyaluronic Acid the Best Injective Treatment for Grade II-III Knee Osteoarthritis? A Prospective Study.

The results of this study suggest that a combination injection of HA and PRP can significantly improve pain compared to HA or PRP alone for patients with grade 2-3 knee osteoarthritis. Improvements in functional outcomes were superior in the combination group, and may have been affected by the small sample size. A larger scale study exploring a variety of objective and subjective clinical outcomes is of interest.

60 adults with grade II-III knee osteoarthritis were randomized to receive an intra-articular injection of hyaluronic acid (HA; n=20), platelet-rich plasma (PRP; n=20) or HA+PRP (n=20). Outcomes of interest included pain on a Visual Analog Scale (VAS) and Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) scores, measured at 3 and 6 months post-treatment. Combination injection significantly improved pain scores at 6 months compared to both individual components; statistically significant differences in WOMAC scores were not observed, but the combination group at the best scores and the lack of statistical significance may have been due to the small sample size.
