Precise Acetabular Prosthesis Positioning Using a Novel Robot-Assisted Total Hip Arthroplasty System: A Multicenter, Randomized Controlled Trial.

A total of 138 patients undergoing primary THA were randomized to receive either robotic-assisted THA using the novel “Ibot” system (n=69) or conventional THA (n=69). The primary outcome was the proportion of acetabular prosthesis placement within the Lewinnek safe zone for anteversion and inclination angles. Secondary outcomes included operation time, WOMAC score, Harris score, SF-36 health questionnaire, hip dislocation rate, and adverse events. Outcomes were assessed at 6 ± 2 weeks postoperatively. Overall, the study revealed a significantly higher proportion of acetabular prostheses positioned within the safe zone in the robotic group (92.2%) compared to the conventional group (50.8%). The robotic-assisted THA resulted in a significantly longer operative time but showed no significant differences in functional scores, dislocation rates, or adverse events. These findings suggest that the robotic system significantly enhances prosthesis placement accuracy while maintaining comparable safety and functional outcomes.
