Ibogaine: complex pharmacokinetics, concerns for safety, and preliminary efficacy measures

Ibogaine was shown to decrease cravings and subjective depression from baseline in patients undergoing detoxification from cocaine and heroin. Ibogaine also showed rapid clearance from the blood, so the effects may be related to noribogaine which is cleared much slower. Future studies with larger sample sizes and randomized designs with comparator arms are warranted.
Study Summary
Twenty-seven treatment-seeking opioid and cocaine dependent participants were assigned to one of three (500mg, 600mg, 800mg) doses of ibogaine in an open-label case series study. Opioid and cocaine craving, Beck Depression Inventory (BDI), and serum concentrations of ibogaine and noribogaine were examined. Ibogaine significantly decreased craving for cocaine and heroin during detoxification, and also showed lower BDI scores 30 days after detoxification. Biochemical analysis showed that ibogaine is cleared rapidly from the blood, but levels of noribogaine can remain elevated much longer.
