Parathyroid Hormone in the Management of Pelvic Fragility Fractures: A Systematic Review and Meta-Analysis.

6 studies including 188 patients with fragility fractures of the pelvis were included in this systematic review and meta-analysis comparing parathyroid hormone (PTH) therapy vs standard care, placebo, sacroplasty, or internal fixation. Pooled outcomes of interest included radiographic fracture healing, time to union, visual analogue scale (VAS) pain scores, adverse events, and complications. Overall, most studies reported faster fracture healing and lower pain scores with PTH treatment. Meta-analysis of three studies demonstrated a pooled reduction in pain favouring PTH, although this did not reach statistical significance. The findings suggest that PTH may accelerate healing and improve pain in patients with pelvic fragility fractures, though the evidence base is limited by small sample sizes and substantial risk of bias.
