Comparing the Outcomes of Cast Immobilization with and Without K-Wire Fixation for Displaced Distal Radius Fractures in the Pediatric Population: A Systematic Review and Meta-Analysis.

Twelve studies including 1,455 pediatric patients with displaced distal radius fractures were included in this systematic review and meta-analysis comparing cast immobilization alone versus cast immobilization with K-wire fixation. Pooled outcomes of interest included re-displacement, secondary surgery, complications, range of motion, and radiological measures such as angulation. The meta-analysis showed that K-wire fixation was associated with significantly lower re-displacement and reduced need for secondary surgery, while complications were comparable between groups. These findings suggest that K-wire fixation offers superior stability and reduces the likelihood of reoperation, although both approaches have similar complication profiles.
