Does lateral extra-articular tenodesis affect knee stability in cases with isolated anterior cruciate ligament reconstruction?

Forty patients with isolated ACL injuries were randomized to receive either anatomic single-bundle ACL reconstruction (ASB-ACLR) with iliotibial band tenodesis (LET) (n=20) or ASB-ACLR alone (n=20). The primary outcome of interest was knee stability, assessed by the pivot-shift test. Secondary outcomes included Lysholm knee score, Lachman test results, and postoperative complications. Outcomes were evaluated preoperatively and at 9 months postoperatively. Overall, the study found that ACLR with LET significantly reduced the incidence of high-grade pivot-shift phenomena (p = 0.003), but no significant differences were observed between groups in Lysholm scores (p = 0.832) or Lachman test results (p = 0.106). These findings suggest that LET improves rotational stability but does not significantly enhance subjective knee function compared to ACLR alone.
