Kirschner's Wire versus Casts in Wrist Fractures: A Systematic Review and Meta-analysis.

Study Summary

Nine studies totalling 1097 patients with distal radius fractures were included in this systematic review and meta-analysis comparing Kirschner wire (K-wire) fixation versus plaster cast immobilization. Pooled outcomes of interest included range of motion (ROM), grip strength, patient-reported outcome measures (PRWE), complication rates, and malunion. The meta-analysis demonstrated significantly higher rates of total adverse events and malunion in the cast group, along with a significant difference in PRWE scores at 12 months favouring K-wiring. However, no significant differences were observed in ROM or grip strength at 12 months. Overall, these findings suggest that K-wire fixation may provide superior outcomes than cast immobilization in managing distal radius fractures.
