Effect of intramedullary platelet-rich plasma on fracture healing in elderly intertrochanteric fractures: A randomized controlled trial.

84 elderly patients with isolated low-energy intertrochanteric femoral fractures were randomized to receive intramedullary platelet-rich plasma (PRP) injection or placebo saline injection during cephalomedullary nail fixation. The primary outcome of interest was fracture healing assessed using the Radiographic Union Score for Hip (RUSH). Secondary outcomes included postoperative bone turnover markers, Harris Hip Score (HHS), Barthel Index, and De Morton Mobility Index (DEMMI). Outcomes were assessed at 2 weeks, 6 weeks, 3 months, and 6 months postoperatively. Overall, the results of the study revealed that the PRP group demonstrated significantly improved early radiographic healing at 2 weeks. No significant differences were observed in fracture healing, bone turnover markers, or functional recovery at later follow-up periods. These findings suggest that intramedullary PRP may enhance very early fracture healing but does not provide sustained clinical or functional benefit.
