Individualised compared to off-the-shelf total knee arthroplasty results in lower and less variable patellar tilt.

Two hundred eighty-nine patients with end-stage osteoarthritis undergoing primary total knee arthroplasty were randomized to receive either individualised TKA (n = 142) or off-the-shelf (OTS) TKA (n = 147). The primary outcome of interest was absolute postoperative patellar tilt. Secondary outcomes included variability in patellar tilt across coronal alignment phenotypes, patellar height, and intra-/interobserver reliability. Outcomes were assessed at 4 months postoperatively. Overall, the results of the study revealed significantly lower and less variable postoperative patellar tilt in the individualised TKA group. These findings suggest that individualised TKA may offer improved alignment of the patellofemoral joint independent of preoperative phenotype.
